Primary Health Networks

Primary Health Networks (PHNs) are a federal government health initiative, established in 2015 with the key objectives of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes and improving coordination of care to ensure patients receive the right care in the right place, at the right time. Each of the 31 PHNs across the country are controlled by a board of medical professionals and advised by a clinical council and community advisory committee.

The Australian Capital Territory Primary Health Network covers the entire area of the ACT. This PHN is also known as the Capital Health Network (CHN) and further information and news updates are published regularly via the CHN website. There are ten Primary Health Networks across New South Wales:
Central & Eastern Sydney PHN 
The Northern Sydney Primary Health Network includes the region from Strathfield to Sutherland, as far east as Bondi, and also includes Lord Howe Island and Norfolk Island.

Northern Sydney PHN The Northern Sydney Primary Health Network includes Avalon, Glenorie, Hornsby and Manly.

South Western Sydney PHN The South Western Sydney Primary Health Network includes Bargo, Bundanoon, Campbelltown, Liverpool, Mittagong, Rossmore and The Oaks.

Western Sydney PHN The Western Sydney Primary Health Network includes Kellyville, Mount Druitt and Parramatta.

Hunter New England & Central Coast  PHN The Hunter New England and Central Coast Primary Health Network includes Armidale, Bulahdelah, Cessnock, Forster, Glen Innes, Gosford, Gunnedah, Inverell, Moree, Muswellbrook, Narrabri, Nelson Bay, Newcastle, Quirindi, Tamworth, Taree and Tenterfield.

Murrumbidgee PHN The Murrumbidgee Primary Health Network includes Deniliquin, Griffith, Gundagai, Hay, Narrandera, Tumbarumba, Wagga Wagga and Young.

Nepean Blue Mountains PHN The Nepean Blue Mountains Primary Health Network includes Colo, Katoomba, Lithgow, Penrith, Portland, Richmond, Springwood, St Albans and Windsor.

North Coast NSW PHN The North Coast Primary Health Network includes Ballina, Casino, Coffs Harbour, Dorrigo, Evans Head, Grafton, Kempsey, Lismore, Murwillumbah, Nambucca Heads and Port Macquarie.

South Eastern NSW PHN The South Eastern NSW Primary Health Network includes Cooma, Eden, Goulburn, Nowra, Ulladulla, and Wollongong.

Western NSW PHN The Western NSW Primary Health Network includes Balranald, Bourke, Brewarrina, Broken Hill, Cobar, Condobolin, Coonabarabran, Forbes, Grenfell, Lightning Ridge, Oberon, Orange, Walgett and Wilcannia.

The Northern Territory Primary Health Network covers the entire area of the NT with two dedicated offices in Darwin and Alice Springs. Resources, news updates and further information can be accessed via the NTPHN website.

There are seven Primary Health Networks across Queensland:
Brisbane North PHN 
Supporting more than 900,000 people, the Brisbane North PHN covers communities in Brisbane's northern suburbs, the Moreton Bay Regional Council and parts of the Somerset Regional Council.

Brisbane South PHN The Brisbane South Primary Health Network supporting an area including Moreton Island, North Stradbroke, Birkdale, Victoria Point, Daisy Hill, Bay Islands, Browns Plains, Beenleigh, Greenbank, Tamborine, Beaudesert, Kooralbyn, Lamington and Mt Barney.

Darling Downs and West Moreton PHN With two offices in Toowoomba and Ipswich, the Darling Downs and West Moreton PHN supports communities in Ipswich and Toowoomba, plus the surrounding rural communities of the Lockyer Valley, Boonah (Scenic Rim), Esk (Somerset), South Burnett, Southern Downs, Goondiwindi and the Western Downs.

Gold Coast PHN The Gold Coast Primary Health Network includes Ormeau/Oxenford, Robina, Southport, Surfers Paradise, Broadbeach/Burleigh, Coolangatta, Gold Coast North, Gold Coast Hinterland, Mudgeeraba/Tallebudgera and Nerang.

Central Queensland, Wide bay and Sunshine Coast PHN The Central Queensland, Wide Bay and Sunshine Coast Primary Health Network includes the areas of the Glasshouse Mountains, north of Yeppoon, west beyond Emerald, as well as the areas of Rockhampton, Hervey Bay, Maryborough, the Sunshine Coast Hinterland, Gayndah, Gladstone and Bundaberg.

North Queensland PHN The North Queensland Primary Health Network includes Mackay, Townsville, Cairns, Cape York Peninsula and Torres Strait regions.

West Queensland PHN The West Queensland Primary Health Network supports communities in the areas of the Northwest, Central West and South West Queensland state Hospital and Health Services (HHSs). These regions include Mornington, Carpentaria, Doomadgee, Burke, Mount Isa, Cloncurry, McKinlay, Boulia, Winton, Diamantina, Barcoo, Longreach, Barcaldine, Blackall Tambo, Quilpie, Bulloo, Paroo, Balonne, Maranoa and Murweh.

Adelaide (Metropolitan South Australia) PHN The Adelaide PHN supports communities in Aberfoyle Park, Adelaide, Aldinga Beach, Angle Vale, Bedford Park, Blackwood, Brighton, Craigmore, Daw Park, Elizabeth, Enfield, Glen Osmond, Glenelg, Grange, Hallett Cove, Happy Valley, Henley Beach, Highbury, Magill, Marion, Maslin Beach, McLaren Vale, Modbury, Munno Para, Noarlunga Centre, Norwood, Osborne, Para Hills, Parafield, Plympton, Port Adelaide, Reynella, Salisbury, Seaford, Sellicks Beach, Semaphore Park, Smithfield, Unley, Virginia, Willunga and Woodville.

Country South Australia PHN The Country SA Primary Health Network includes Berri, Ceduna, Clare, Coober Pedy, Gawler, Loxton, Millicent, Mount Barker, Mount Gambier, Murray Bridge, Naracoorte, Port Augusta, Port Lincoln, Port Pirie, Renmark, Roxby Downs, Streaky Bay, Tumby Bay, Victor Harbour, Wallaroo and Whyalla.

Primary Health Tasmania is the PHN that supports entire state of Tasmania. Consumers and Service Providers can access resources, news updates and further information via the Primary Health Tasmania website.

The Victorian PHN Alliance is a collaboration of all Victorian Primary Health Networks to share knowledge, resources and learning opportunities to better support all Primary Health communities throughout Victoria.

Eastern Melbourne PHN The Eastern Melbourne Primary Health Network covers several local governments including the City of Banyule, City of Boroondara, City of Knox, City of Manningham, City of Maroondah, Shire of Mitchell, City of Monash, Shire of Murrindindi, Shire of Nilumbik, City of Whitehorse, City of Whittlesea and the Shire of Yarra Ranges.

North Western Melbourne PHN The North Western Melbourne Primary Health Network includes Brimbank, Hume, Macedon Ranges, Melbourne, Melton, Moonee Valley, Moreland, Maribyrnong, Wyndham and Yarra.

South Eastern Melbourne PHN The South Eastern Melbourne Primary Health Network covers communities from St Kilda to Sorrento to Bunyip, including the major population hubs of Clayton, Dandenong, Moorabbin, Caulfield, Cranbourne, Frankston and Pakenham.

Gippsland PHN The Gippsland Primary Health Network includes the six Gippsland Local Government Areas of South Gippsland, Bass Coast, Baw Baw, Latrobe City, Wellington and East Gippsland Shires.

Murray PHN The Murray Primary Health Network includes North West Victoria (Mildura, Robinvale, Swan Hill and Donald), Central Victoria (Kerang, Echuca, Bendigo, Castlemaine and Kyabram), the Goulburn Valley (Cobram, Yarrawonga, Shepparton and Seymour) and North East Victoria (Albury, Wodonga, Wangaratta, Benalla and Mansfield).

Western Victoria PHN There are four regional centres in the Western Victorian Primary Health Network in Geelong, Ballarat, Horsham and Warrnambool to support the areas of Hindmarsh, Yarriambiack, West Wimmera, Horsham, Northern Grampians, Central Goldfields, Pyrenees, Ararat, Southern Grampians, Glenelg, Moyne, Hepburn, Ballarat, Moorabool, Golden Plains, Warrnambool, Corangamite, Colac-Otway, Greater Geelong, Queenscliffe and the Surf Coast.

The WA Primary Health Alliance is a collaboration of all WA Primary Health Networks as a mechanism for integrating services across organisations and across boundaries.

Perth North PHN The Perth North Primary Health Network covers the north and north eastern areas of the Perth metropolitan area which includes Perth City to Joondalup and East through the Midland/Swan region.

Perth South PHN The Perth South Primary Health Network covers the south and south eastern areas of the Perth metropolitan area including areas from the Swan River down to Mandurah, Murray and Waroona and east to Armadale/Pinjarra.

Country WA PHN The Country WA Primary Health Network consists of several regions including Goldfields, Great Southern, Kimberley, Midwest, Pilbara, South West and the Wheatbelt.
